Basic Interview Setup package

I'm looking to hire a reliable videographer / cinematographer with their own interview setup package (camera/lighting/sound) package. I will be interviewing a family members of a gentleman who was killed in an accident. To be shown to the insurance company, perhaps a jury. This month, the weekend of the 17th. Should be a one day gig. Specific day / time TBD.

Camera: 4k with lens capable of capturing a shallow depth of field. Sound: Ideally boom out of frame, but willing to consider lavs. Lighting: Nothing fancy. Soft, pleasant lighting on subject with background (the room of a house) lit.
